Please use the two forms below to submit new calling or release proposals to the Stake Presidency or notify them of planned unit callings that they should be aware of.

Recommendations for Patriarchal Blessings, Temple Ordinances/Attendance, Melchizedek Priesthood Ordinations or Missionary Service should be submitted via the appropriate form in Leader and Clerk Resources (LCR).
